Developing a “Culture of Safety” A year ago we wrote about the complex process involved in identifying, categorizing, and assessing medical error in health care in the United States (”Assessing Medical Error in Health Care,” Health Progress, November– December 2001, pp. 14-17). That article discussed the 1999 Institute of Medicine (IOM) report on medical error, “To Err Is Human,” and the erosion of public trust in U.S. health care.

He learns to depend on the physician in sickness and in health. In its May issue, Export Times published its most comprehensive “UK-World Trade Review (Jan-Dec 1999)” which shows how the UK exports and imports to and from 191 countries in descending order of value. In scanning this enormous list it is noticeable that Canada has jumped to twelfth place with our sales reaching L2556m, but there were also large reductions in UK exports to Italy, Sweden, Hong Kong, Norway, Saudi Arabia, South Africa and Turkey. However, sales to the USA, the UKs top individual export market, increased to L24,373m. Its reporter John Aczel says that sales of British goods in overseas markets have been fluctuating and were slightly easier in 1999. Some improvement is expected over the next 12 months, but much will depend on the level of sterling. The major improvement in 1999 was the recovery in Asia, where a growth of about 2.8 per cent was seen. Sluggish trading continued in Japan, but in other countries, including China, Taiwan, Hong Kong and South Korea economic conditions improved.
Why sell overseas? I am often asked the question “Why export?” In the UK, businesses are subject to much competition and price cutting in the current climate is intense. However, there are approximately 230 countries overseas where your products or service may be vital. At a time when the FSA reports that the level of personal debt threatens the economy, and assuming that property prices fall and domestic demand declines, the only way to increase sales and boost profit is to export. AN ITALIAN who claims to be a qualified criminal lawyer was asked yesterday to explain himself to three Court of Appeal judges. Giovanni di Stefano, who has represented some of Britains most notorious criminals, including Kenneth Noye, Jeremy Bamber and Harold Shipman, was questioned by the court over his right to practise law in Britain. Mr Lynch replied: “Mr Di Stefano. I dont believe he is a representative of Paul Martin.” But the judge said that Mr Di Stefano, who said he worked for Italian law firm Studio Legale Internazionale, had signed written legal advice in connection with the appeal.
One of Brian McNamees lawyers said Sunday he believed the Justice Department will open a criminal investigation into Roger Clemens denials of doping. Meantime, the chairman of a congressional committee said comments attributed to one of the pitchers lawyers could be interpreted as trying to intimidate a federal law enforcement official. “I think there will be a criminal prosecution after Wednesday, and that means there will be grand jury proceedings and subsequent proceedings,” said Richard Emery, one of McNamees attorneys. “I dont see theres any possibility that Brian has any jeopardy. I only see the possibility of Clemens getting investigated by Justice, whether or not Congress refers it.”

The earnings of some top barristers (report, 15 September) represent a tiny proportion of practitioners. These are not remotely relevant to the debate about low fees. The Bar have always suspected that those who serve government with their black art of spin would release these figures now. They are fees approved by the government- sponsored Legal Services Commission and that the Government have known about for months.

The Associated General Contractors and state and federal officials signed a landmark statewide safety partnership on Wednesday. We look forward to a successful partnership and a long relationship, said AGC Executive Vice President Bob Barker moments before the pact was inked at the AGC headquarters in Madison. Kim Stille, OSHA area director in Madison, said the agency has long worked productively with AGC members, but this partnership ratifies the commitment of both contractors and government safety officials to prevent and reduce injuries.
The Milwaukee Occupational Safety and Health Administration office Wednesday signed a safety partnership with CG Schmidt Construction for GE Healthcares new Wauwatosa headquarters. The agreement is the second between OSHA and CG Schmidt. The first was on the 875 East Wisconsin office building project where the injury and illness rate was half of the state average. It had six recordable injuries during the projects 188,384 man-hours. GE Healthcare also has a solid safety reputation with OSHA based on the numerous voluntary protection program agreements it has with the agency. Four months ago, CG Schmidt broke ground on the $87 million headquarters, which GE is developing through a partnership with Irgens Development Partners LLC. It is expecting to finish the project in January 2006. Under the agreement, CG Schmidt will enforce safety and training programs that exceed OSHA regulations on the project and in return will have reduced penalty payments and a guarantee of one unannounced site inspection a year. It requires CG Schmidt to maintain a lost- time injury and illness rate 25 percent below the Wisconsin construction industry average.
PRESIDENT BUSH signed a resolution repealing OSHAs ergonomics rules on March 20, 2001. On March 8, the U.S. House of Representatives passed a resolution of disapproval of OSHAs final ergonomics rule by a vote of 223 to 206. Two days earlier, the Senate had passed the same resolution 56 to 44. Voting in both houses ran closely along party lines. Sixteen Democrats in the House joined all but 13 Republicans in voting for passage. In the Senate, six Democrats joined all 50 Republicans in voting to approve the resolution.

SHOE shops are selling thousands of children badly fitting footwear, a report reveals. Parents feel they are getting a poor deal from stores which have axed their fitting service. The poll of 500 parents by shoe chain Brantano found more than a third of children lied their shoes fit correctly and one in five chose their own footwear - often leading to rows.

A fully functional hospital syndromic surveillance system that uses automated analysis (such as the daily emergency department-based surveillance with SaTScan in New York City) should identify a substantial increase in a relevant syndrome within 12 to 24 hours after data submission (2). A continued daily rise in any disease category would most certainly set off alarms in a syndromic surveillance network. If active statewide laboratory surveillance is included in syndromic surveillance, such as the gram-positive rod surveillance conducted in Connecticut (3), this surveillance should rapidly detect even single cases of anthrax concurrent with the presumptive diagnosis within the hospital. The authors also state that syndromic surveillance would not detect outbreaks too small to trigger statistical alarms. The combination of active and passive surveillance in the hospital admissions based syndromic surveillance in Connecticut allows a number of syndromes to be tracked immediately upon notification; these syndromes include pneumonia and acute respiratory disease in healthcare workers admitted to a hospital, all disease clusters, and fever with rash illness. This system is very flexible, and active surveillance of other syndromes can be quickly instituted as required. This active surveillance component has been proven useful. The first 2 of Connecticuts 17 confirmed human cases of West Nile virus during 2002 were discovered in August when a health director, who regularly monitored the syndromic admissions data for the hospital in his municipality, requested immediate West Nile virus testing from the hospitals infection-control department when he received two late summer reports of neurologic illness. Buehler et al. state that specificity for distinguishing bioterrorism-related epidemics from more ordinary illness may be low because the early symptoms of bioterrorism-related illness overlap with those of many common infections. Illness specificity can be modulated within a syndromic surveillance system by making changes in the definition of the information requested, the method of analysis used, or by incorporating varying amounts of active surveillance into a passive reporting system. In Connecticut, annual rates of hospital admissions for pneumonia and respiratory illness have significantly increased (gt;3 standard deviations) during winter months. These increases have corresponded temporally with peaks in laboratory-confirmed influenza reports and in our state-based and the national sentinel physician influenzalike illness reports. Similarly, in the military-based syndromic surveillance system, respiratory outbreaks are detected by monitoring routine outpatient visits and pharmacy prescriptions. Absolute numbers of visits, as well as percentage of visits, to primary care clinics for influenzalike illness provide up-to-date information on respiratory disease conditions at military installations in both active-duty personnel and family members.
British Aerospace Defence Systems has won two Royal Navy surveillance radar upgrade contracts, worth almost #30m. The second contract will provide new-generation Track Extractor software in all T996 radars. Paul Hoffman, a deputy assistant secretary with the Department of the Interior, is looking to take Yellowstone Park off the list of “endangered cultural sites” kept by the United Nations. Yellowstone was placed on the list in 1995 when the area was threatened by a proposed New World gold mine. Hoffman said the gold threat is now gone, therefore the distinction is no longer needed.
In 2004, the EMS team identified its first large-scale initiative–solid waste management. Some of the guidelines introduced include: * Recycling opportunities for visitors. There are currently 63 recycling bins in the park, and it is estimated that there has been a 50 percent increase in the amount of commodities collected in the park, particularly plastics. * Recycling propane cylinders. Through a partnership with Grand Teton National Park, Yellowstone and Grand Teton concessionaires, the Yellowstone Park Foundation and various private entities, there is now a program of collecting and recycling propane cylinders in the greater Yellowstone area. The Propane Bottle Recycler (PBR), a mobile propane recycling unit, is now being used to recycle the estimated 3,000, one-pound propane cylinders discarded in the park each year.
YELLOWSTONE NATIONAL PARK, Wyo. — Geothermal activity is increasing in a Yellowstone National Park geyser basin and the bottom of Yellowstone Lake is bulging, but scientists say there is no impending major eruption. Searing ground temperatures, bursts of steam and flows of hot water prompted park officials to close about half the trails in Norris Geyser Basin this summer, and the increased thermal activity has led to concern among some visitors about the potential for an eruption. But scientists say there is no evidence that Yellowstone is poised for a powerful eruption, such as the one that rocked the region 600,000 years ago.

A specialty furniture manufacturer and retailer will open a showroom this fall in the Village Park Shopping Center, filling part of the vacancy left by the June 1 closure of Bollingers Books. Dallas-based Leather Center is spending about $50,000 to renovate 5,000 square feet at 10505 N. May Ave. Leasing agent Carl Archiniaco, vice president of Harrison Levy Co., said he will target other home improvement merchants to fill the shopping centers vacancies.
